I confirm that I have practiced a minimum of 1000 hours (approximately 6 months full-time experience) within the last two years as a neonatal Occupational Therapist, Physical Therapist, Speech Language Pathologist, Registered Nurse or MD/DO in the Neonatal Intensive Care Unit or Special Care Nursery. I am in good standing with my professional licensing body, and have a current professional license.
